If you are unable to keep your appointment, please let us know by phone and/or email 48 hours in advance. This allows us to schedule another person.
At the time of scheduling your Global Assessment appointment, payment will be due in full as we block this time exclusively for you. Payments for the Global Assessment are non-refundable.
FEES FOR LAB AND OTHER STUDIES
Lab, sleep study, and other testing fees are not included in the membership fee or the Global Assessment fee because these studies are conducted by parties outside of LWmed. For example, lab fees will be due to the laboratory, not to LWmed. Your insurance might cover some or all of the testing fees, so remember to check with your plan.
Because research shows that weight loss and long-term maintenance improves with a long-term commitment, many people want to automatically renew their membership. By doing so you will make sure to keep your spot in our program. If you choose not to continue with automatic renewal, we encourage you to team up with us again when you are ready -- if six months or more have gone by since your membership expiration then all you have to do is make an appointment for a Global Assessment Update. That way we'll have the most accurate information about you to provide you the best care. And we'll be on our way partnering together for a healthier you.
